Make a Referral

Who can refer?


Currently our service is funded to support people in Nottingham City who are in or leaving secondary mental health services. Normally a person in secondary mental health services would have a Community Psychiatric Nurse (CPN) or a Psychiatrist.

At present we are not funded to support people referred by GPs, IAPT services or self referrals.

If you are supported by a CPN or Psychiatrist and would like support from us, please ask them to get in touch with us.

Download the referral form at the bottom of this page. Please provide as much detail as you can, and any risk assessments you may have undertaken with the person.

Please password protect the document when sending, to ensure the information is kept securely. The password can be sent separately.

> What happens after I send the form?

We will email to confirm that we have received the referral and let you know if there will be a wait for the service.

When we are able to allocate a support worker to the case, they will contact you to make you aware and to get any more information if they need to.

> I want to refer someone who is in secondary mental health services, but lives outside of Nottingham City. Can I make a referral?

Unfortunately, we are only funded to support people in Nottingham City at present.

> How frequently does your support take place?

All of our support is tailored to an individual’s needs and goals. However, given our capacity, we are not able to provide multiple visits a week and we are not a substitute for a care package.

Our virtual groups currently take place fortnightly. We hope to restart our face-to-face groups as soon as we can: four groups run once a month, meaning people we support can attend a different group every week.
