Make a Referral

Who can refer?


Currently our service is funded to support people in Nottingham City diagnosed with a Severe Mental Illness and who are also experiencing two other intersecting forms of Severe Multiple Disadvantage such as Domestic Abuse/ Sexual Violence, harmful Substance Use, significant risk of Homelessness and/or contact with the Criminal Justice System. Support may include, but is not limited to:

  • Making benefit applications.

  • Managing finances/ budgeting.

  • Liaising with Adult Social Care.

  • Providing digital support.

  • Link in/refer to other services in the community.

  • Access to regular peer social groups.

  • Access to a voluntary befriender where appropriate

We are accepting referrals for people who are receiving support in primary care services, from their GP or those in ARRS roles. Please note, that we cannot accept self referrals or referrals for people in secondary mental health services.

We will email to confirm that we have received the referral and let you know if there will be a wait for the service.

When we are able to allocate a support worker they will contact you to make you aware and to get any more information, if this is needed.

> I want to refer someone who is in primary mental health services, but lives outside of Nottingham City. Can I make a referral?

Unfortunately, we are only funded to support people in Nottingham City at present.

> How frequently does your support take place?

All of our support is tailored to an individual’s needs and goals. However, given our capacity, we are not able to provide multiple visits a week and we are not a substitute for a social care package.

Our groups take place regularly: seven groups run once a month. meaning people we support can attend a different group every week.
